We have been on a couple of Mediterranean cruises - one from Southampton, England to the western Med, the other from Venice to the Eastern. Both were on NCL as we have earned a high level with their loyalty program and were able to sail in suites which was wonderful. Our stops included Barcelona, Rome,Malaga and Vigo, Spain, Cannes, Gibraltar and Lisbon. We really enjoyed all stops with Lisbon and Malaga (tour to Ronda) being our favorites. We also stopped in Livorno, Italy and took a tour to Cinque Terre - this is a must-do!
Last summer we cruised from Venice to Split and Dubrovnik, Croatia, Ephesus Turkey, and several Greek islands including Mykonos, Santorini and others. Dubrovnik, Mykonos and Santorini were spectacular and we really enjoyed those ports.
We tend to try to do shore excursions independently as they usually cost less and you are able to go with a smaller group. When the itinerary is tight or the place we want to see is fairly far from port, we will do a ship's excursion just for the peace of mind.

I used to work for Princess Cruises and cruised all through the Med. I loved cruises that started in Venice. It depends on what you want Greek Islands are good for the beach and Italy, Spain and France have great cities.
